Follow the Step by Step Process:
1. Meet with the Veteran and/or Spouse's doctor to complete the Physician Statement.
3. Collect the Necessary Collateral Materials for the Application listed on the Checklist. Note: If you do not have the original discharge papers you may complete a Standard Form 180 and request a verification of the service.
